Please download the PDF and save it before emailing. Completed applications can be emailed or mailed. 
Please allow at least 72 hours to process completed applications, as we run completely on a volunteer basis. 
​To help expedite this process, contact your current vet and give them permission to speak with us.

Please note that submitting an application is not a guarantee of adoption.
Adoptions are not “first come first serve”, they are the best fit for the dog.

*Senior Dogs 7 years or older: $80.00

Vet Exam
Distemper/Parvo (DHLPP)
Rabies Vaccine (12 weeks and up)
Bortdetella Vaccine
Deworming
Heartworm testing (6 months and up) 
Microchipping 
Flea/Tick & Heartworm preventative (while under care of SC) 
Spay/Neuter
